Provide your name, address, telephone number, and email address.

Include a good-faith statement that the disputed use is not authorized by the copyright owner, its agent, or the law.

Include a statement under penalty of perjury that your notice is accurate and that you are authorized to act on behalf of the owner.

If your content is removed due to a copyright complaint, you may submit a counter-notice if you believe removal was in error.

Counter-notices should include your signature, identification of removed material, a statement under penalty of perjury, and consent to jurisdiction as required by applicable law.
